working with word

I am sorry if this is not the place to ask this question but if I was hopping that someone could shine some light on this for me. I am trying to create a form in word that you select from 2 or more things and then based on the things you checked some sort of macro or script will make a paragraph from what you checked. I will try to give a example.

   

and the output from the macro or script would be

hello jack how was your trip?

I need this to be in word and I am assuming that words macro or forms or script will do it but I have no clue.

Why does this have to be in word? QM can do it really well with a dialog.

For my wife’s work. All other forms are in word and introducing another program for what I hope is such a small task may not be such a good idea.
