Toolbars aren't loading anymore

When I log out of Windows 7 and log back in toolbars don't load to apps (Outlook and Excel) when I open them.

I have restarted QM and restarted the apps but they don't load

If I reboot, it fixes it

This has been working flawlessly for many months but stopped working about a week ago

And all window triggers don't work then? For example when you assign a window trigger (the same as assigned to a toolbar) to a macro or function. Keyboard and mouse triggers too?
How if you assign a different window trigger, for example "window active" or "window visible"?
And QM window then does not have the floating toolbar (Text, Keys, Mouse...)?
Also look in QM "Running items" pane. Maybe these toolbars are loaded, but somehow invisible.
Windows 32-bit or 64-bit?
QM 2.4.2?

KB and mouse triggers work
The floating toolbar in QM works
If they are invisible will they still work? Clicking the area where they should be does nothing.
I enabled Running Items in Options but I don't see any pane showing it.
Windows 7 64 bit
QM v2.3.3.7

Should I update before going further? How is that done? I don't see a "Check for update" entry in QM

Running items are in the right side of QM window. If hidden, click menu Run -> View active items. Don't need to enable in Options.
